We collect appointment and contact details you submit, transactional-message consent records, and limited security logs. We do not ask you to submit detailed medical histories online.
How information is used
We use this information to provide and manage appointments, answer inquiries, send requested transactional notices, protect the service, and meet legal obligations. Service providers may process data only to perform those functions.
Retention and choices
